Key Requirements

You must be prepared with:

☑ Completed Passport Application Form

☑ Current Sri Lankan Passport (if available)

☑ Original Birth Certificate

☑ National Identity Card (NIC) (if applicable)

☑ Recent passport-size photographs (as per specifications)

Additional Documents (if applicable):

☑ Marriage Certificate (for name changes)

☑ Dual Citizenship Certificate

☑ Police report (for lost or damaged passports)

☑ Supporting documents for any changes in personal details

Important Conditions

⚠ Bring all original documents + 1 set of A4 photocopies

⚠ Passport copies must include bio-data and observation pages

⚠ Your name and details must exactly match official records

⚠ Translations are not accepted in place of originals

⚠ Incomplete documentation may result in delays or rescheduling
